Nurs 492 – Community Health Nursing  

Purpose of Assignment:

The purpose of this assignment is clinical application project paper is for students to assess the health care needs of a population within their community.

Student Approach to Assignment:

  o I used a personal and professional approach when choosing the topic for this project. Personally I have family members affected by the health concern discussed in the paper, and professionally I have care for many patients with this health issue. From this assignment, I learned so much more about the community in which I live in relationship to this particular health concern.

 

Reason for Inclusion of this Assignment in the Portfolio: 

 o This is assignment exemplifies how nurses can help in the community. The assignment made me step outside of my comfort zone (which is hospital nursing) and allowed me to really understand the importance of community health and how it correlates with many different types of nursing. 

 

• Leadership

o Initiates community partnerships to establish health promotion goals and implements strategies to meet those goals

For this assignment I partnered with Broad Creek Community Center in hopes to help with the prevention of childhood obesity. Strategies used were teaching about healthier eating and fundraising for healthier snacks in the community center as well as toys that allowed the children to be more active.

o Organizes, manages, and evaluates the development of strategies to promote healthy communities

   I organized the partnership with the community service coordinator at Broad Creek Community Center, created posters to display healthier eating options and recommendations for physical activity, tracked the login sheet for children who used the toy that allowed more physical activity (i.e. children would sign out jump ropes and once they were done they would sign them back in), and evaluated the children’s understanding of the importance of healthier eating and getting physically active each day.

• Research 

o Shares research findings with colleagues

In this clinical application paper there are many data found via research stating importance of physical activities and healthier diet to prevent childhood obesity which in turn can reduce further diseases as an adult such as type II diabetes and heart disease.  

• Teaching

  • Evaluates the efficacy of health promotion and education modalities

Participants for the aggregate community were evaluated for understanding the purpose project as well as importance of project and its long-term health effects.

• Culture

  • Maintains an awareness of global environmental factors that may influence the delivery of health care services

Lack of access to health care is a global environmental factor that influences the delivery of healthcare services to this aggregate community. In the paper I state the lack of access to healthcare and the importance of preventative medicine for this purpose.

          o Integrates knowledge of cultural diversity in performing nursing interventions

For this clinical application project it was important to assess the community first, rather than assuming what they may need. This intervention demonstrates cultural diversity by not performing interventions based on my personal views of the aggregate community but on what they actually need.
