Therapeutic Nursing Intervention

NURS 403 – Transition to BSN practice  

Purpose of Assignment:

The purpose of this assignment is to define a clinical problem, review the literature for appropriate research supported nursing interventions, critique current practice and provide a plan for changing or adapting clinical practice. 

Student Approach to Assignment

I approached this assignment by reviewing common problems in practice I see every day at work. This assignment reflects my personal interest because as a labor and delivery nurse, and as a mother, I feel passionate in being able to aide other mothers/soon-to-be mothers in their ideal delivery as long it is safe. From this assignment, I was able to gain perspective on some ways to help reduce the problem that is discussed in this paper.

Reason for Inclusion of this Assignment in the Portfolio 

 I included this paper in my portfolio because I believe it is important as a nurse and nursing student, to see a clinical problem and try to come up with a solution whether it is traditional or non-traditional. As nurses our practice is evidence-based, understanding the evidence and practicing by those standards guides my practice. This assignment represents my passions as a student and a nurse especially because it is the population I work with directly.

 

Critical Thinking

o Uses decision making skills in making clinical or professional judgements

            In this paper I state that it is frequent for primigravida patients to end up in a cesarean section (c-section). I use clinical judgment skills by gathering all the information about my patient and if a cesarean section is warranted, especially in a non-emergent situation.

o Revises actions and goals based on evidence rather than conjecture 

Reviewing the main reasons a patient goes for a cesarean section and reviewing the evidence to see if there are other routes that can be taken, using the c-section as the absolute last resort. In my paper I state that creating a stop huddle for c-sections might aide in the decrease in c-section rates.  

Nursing Practice 

o Applies appropriate knowledge of major health problems to guide practice 

Multiple research articles were used in my research, most of the articles were within five years. Only one article was greater than five years, but compared to other literature it still holds relevance in the paper. According to Joshi et al. (2021) “caesarean section rate in primigravida was observed to be 29.5%...”, while this may not initially seem like a major health concern, it is because the world consist of men and women, women who are child bearing all could end up facing a c-section and the purpose of this paper is to shed light on the procedure, its consequences, and how to help decrease the rate.

  • Professionalism

    • Differentiates between general, institutional, and specialty-specific standards of practice to guide nursing care

      • In this paper I discuss how in general c-sections usually happen because of many reasons and one being the stall of labor, or labor dystocia. I also discuss that at my place of employment one way we try to reduce the rates of c-sections is constant movement of our patients, especially those who are who have epidurals, in or to aide in the progression of labor, this is also specialty-specific.

  • Leadership

    • Assumes a leadership role within one’s scoop of practice as a designer, manager, and coordinator of health care to meet the needs of the population

      • In this paper I demonstrate being a designer by coming up with the idea to create a stop huddle for c-sections, a manager by overseeing c-sections rates and coming up with solutions to prevent them, and a coordinator of health by working with and my peers to make sure that we all understand the goal that we are trying to achieve.

  • Research

    • Evaluates research that focuses on the efficacy and effectiveness of nursing interventions

      • In this paper I use articles that review literature of c-sections as well as literature that reviews ways to prevent them. An example is when I stated that interventions that practice offering patients alternative pain and nonmedical interventions an aide in reducing c-section rates

    • Applies research-based knowledge from the arts, humanities and sciences to complement nursing practice

      • Research based knowledge from humanities, sciences, and the arts is addressed when Moradi et al. stated that there are factors that affect the education process such as willingness to learn, the skills of the teacher, and the extent to which the teacher addresses all of the appropriate subjects (2019).

  • Teaching
    • Provides teaching to patients and/or professionals about health care procedures and technologies in preparation for and following nursing or medical interventions

      • In this paper I review the procedure of a cesarean section, why women have one, and possible ways to help prevent them from occurring. I also state that there are cases where a c-section is warranted.
